Effective immediately, orders for personal protective equipment (PPE) will need to be submitted using this updated form. This update is part of a streamlined PPE ordering process designed to improve efficiency and inventory management. Only one type of PPE should be submitted per form. Districts who wish to obtain PPE from the Oregon Health Authority should continue to submit purchases through their county emergency managers.
